    摘要：目的：了解胃肠手术抗生素使用和术后感染的情况。方法：对552例胃肠手术患者进行调查，内容包括是否使用抗生素，抗生素的种类，联合应用，应用时机，使用时间及术后感染情况。结果：术后感染发生率为12.5%，在围手术期抗生素的使用率100%，使用的种类多，有抗生素的使用指征占83.7%，有联合应用抗生素指征的占15.0%。结论：胃肠手术中应用抗生素不能很好地预防术后感染，胃肠手术中抗生素的使用仍存在不合理之处。

　　Investigation of Antibiotic Using in Stomach Intestine Operation and Postoperative Infection

　　Abstract：Objective: To investigate the situation of antibiotic using in stomach intestine operation and postoperative infection. Method: 552 patients with stomach intestine operation were investigated, the contents include using of antibiotic, category of antibiotic, combination of antibiotic, using opportunity，persistence time and postoperative infection. Results: Incidence of postoperative infection was 12.5%, usage of antibiotic in operation was 100%,many kinds of antibiotic were used, there was 83.7% patients who had indication of antibiotic and 15.0%patients who had indication of combination of antibiotic. Conclusion: Using antibiotic in stomach intestine operation cann't prevent the postoperative infection well, there is unreasonable factor in usage of antibiotic in stomach intestine operation.

　　预防手术野感染，预防性抗生素的使用是重要措施之一。但是近年来各种因素导致抗生素的广泛使用，甚至是滥用，尤其是胃肠道外科手术。为此我们调查了我院近3年胃肠手术围手术期抗生素使用情况报告如下。

　　1  资料与方法

　　1.1  资料来源：我院2002年1 月至2005 年1 月在普外科胃肠专业组手术并住院患者552例，全部病例均行胃肠道手术。

　　1.2  调查内容：围手术期抗生素使用及感染发生情况。抗生素使用情况包括是否使用抗生素，抗生素的种类，联合应用，应用时机，使用时间。

　　1.3  感染诊断标准：感染包括肺部感染,伤口感染,腹腔、盆腔感染等,经胸片、B 超、诊断性穿刺抽得脓液,血WBC 计数升高等诊断。消化道重建术后吻合口瘘亦归属术后感染病例。

　　2  结果

　　2.1  一般资料：男376例，女146例，年龄19岁～72岁。胃癌根治术75例，胃大部切除术89例，小肠部分切除术145例，全结肠切除术48例，直肠息肉67例及早期癌局部切除术128例。

　　2.2  术后感染的发生率:在调查的552例手术患者中，术后发生感染的人数为69例，感染发生率为12.5%，其构成见表1。

　　2.3  抗生素的使用：在围手术期抗生素的使用率100%，使用的种类多，有抗生素的使用指征462例，占83.7%，有联合应用抗生素指征的83例，占15.0%，使用情况见表2。
